[ Passports: Full Application - Forms PC8 ]
The Application Form
The new Application for an Australian Passport Overseas form (form PC8) must now be used. The form is blue in colour, and has the suffix ‘F’ under the small barcode at the top of each page. Old forms with the ‘E’ suffix will not be accepted after 30 September 2008.

When you obtain an application form, please take some time to read the instructions carefully, particularly the section on the new photograph requirements. We often see clients come to the Passport Office to submit an application who have obviously not read the instructions and do not have all the required information, supporting evidence, or photographs that do not meet the new requirements. New regulations have also been introduced regarding what documents are required to prove your identity. You will be required to provide either an Australian citizenship certificate or a full Australian birth certificate as proof of your Australian citizenship. We have had to turn clients away on many occasions, necessitating sometimes a second and third visit to us, before we can accept the application. |
Application forms are available online from the Passports Online website. Before you visit the website, please note the following:

You can complete and print an application form online, or request an application form be mailed to you. |

If you are applying for an Australian Passport outside of Australia, you should complete an "Application for an Australian Passport Overseas". |

In Hong Kong, application forms are also available from the office of the Australian Chamber of Commerce (Austcham), Qantas Airlines' Office, Tourism Australia Office, the Australian Chinese Association's Office, and the Australian Consulate-General.
In Macau, application forms are available from Catholic Social Services - Macau, Rua da Santa Clara 13, R/C., Macau.
If you would like us to mail an Application Form to you in Hong Kong or Macau, you can send your request (giving us your name, address, and the number of forms required) by fax at 2585 4461, by email to consular.hkng@dfat.gov.au or call us at 2827 8881 during business hours. Please note that we cannot accept forms that are used when applying for a new Passport at a Passport Office in Australia.
Application forms must be completed in black ink and in block letters. As forms are computer read, this system only recognises black ink and block letters. Any other colour ink, form of writing or typing will not be accepted and applicants will be required to complete another application form.
