New Passport Application Forms from 1 July 2008
On 1 July 2008, an new Overseas Passport Application form was introduced to replace the existing 'E' series forms at overseas passport offices. The new forms have the letter 'F' printed underneath the smallest barcode on the application pages. This form is used for renewal for child passport applications, first adult passport applications and for replacement of lost/stolen or damaged passports. The old 'E' forms can be used for passport applications up until 30 September 2008. After this date, only the new application forms will be accepted. Application forms downloaded on or after 1 July 2008 will all be acceptable.
There have also been changes to the proof of citizenship requirements for applications. All applications (except for passport renewals for adults) should be accompanied by an original full Australian birth certificate or an original Australian citizenship certificate.
For applicants who have lost or had stolen their passports, or if the passport is damaged in any way, the new full passport application form will also require the applicant to provide the original of either their full Australian birth certificate or Australian citizenship in support of their application.
Applicants using the new application form who have changed their name after their birth certificate or citizenship certificate was issued will also be required to provide documentary proof of their name change (eg. registered name change certificate or marriage certificate). Previous passports can not be used as proof of name change.
